【IMPORTANT】Declaration Form of Going Abroad

Since we prioritize the health and welfare of all students, we strongly suggest students to refrain from going abroad during summer vacation while facing the increasing confirmed cases of COVID-19 worldwide.

If you really need to leave Taiwan, please fill the Declaration Form (as attached) and get the approvals from your advisor, your department and OIA before you leave Taiwan. Please submit the form at least 2 weeks before your departure day and arrange the quarantine hotel in advance.

Please note that once you decide to leave Taiwan, you need to cover all the quarantine expenses by yourself, including the quarantine hotel fee (Ministry of Education request international students MUST stay in quarantine hotel for 14-day quarantine, and the fee is about NT30,000-NT35,000 for 14 days.), certificate fee of a negative COVID-19 test (if required), transportation fee and etc.  Also, you need to bear all the consequences for the possible risk, including flight cancelation, unable to come back to Taiwan as expected, scholarship suspension and etc.

Thank you for your understanding and cooperation in advance.Your safety and the health will always be our priority. If you have any other concern or question, please feel free to contact us, thank you.
